Community Training Home Coordinator (CTH II Coordinator)

  1. Completing assessments and the Residential Plan based upon those assessments for the initial and annual plan for consumers, attending the annual meeting; scheduling, and chairing called Team meetings (Addendum staffing) as needed to reevaluate consumer’s need, and implementing necessary measures to provide the necessary support to the consumer(s).
  2. Supporting the consumer in his/her efforts toward independent living through development and implementation of training program objectives based upon the Residential Plan, as well as the training and monitoring of staff in the implementation of those objectives.
  3. Responsible to document in monthly progress notes the progress, or lack thereof, of consumer training programs in order to evaluate the effectiveness of the program objectives and/or to revise those objectives as necessary to encourage the consumer to be successful with the programs.  Also responsible to document significant events and any changes in all areas of the consumers’ lives, e.g. medications, weight loss, family contact, etc. within the monthly progress note.
  4. Assisting and supporting staff and consumers with daily concerns and issues, as well as crisis management situations.
  5. Ensuring compliance to all LCDSNB policy and procedures, and DDSN Standards, including the Residential Habilitation Standards and the standards of contract compliance monitored by the state chosen Quality Assurance vendor.
  6. Providing ongoing 24-hour management coverage of the programs, including responsibilities for employee timekeeping procedures, personnel actions, fire and disaster drills, grocery shopping, and other staff related duties.
  7. Monitoring health needs of consumers by assuring medication compliance, and assuring medical appointments are scheduled and conducted as needed.  Also, responsible for investigating any injuries of an unknown origin and reporting those to the Residential Director within an appropriate timeframe.
  8. Responsible for the maintenance of consumer funds at the residence level and for the implementation of the policy regarding consumer funds.
  9. Advocating for consumer’s rights and implementing measures to teach each consumer their rights.
  10. Ensuring that employee evaluations are done timely and that all employees of the area have the current required training.
  11. Carries Agency mobile phone to be able to respond to problems and/or other situations as needed on a round-the-clock basis
  12. Scheduling and chairing monthly staff meetings.
  13. Provides input into the budgeting process and is responsible for staying within the budget established.  Meets quarterly with the Finance Director and Residential Director to review financial status of the area of responsibility.

Bachelor’s degree in a Human Services field and one year of experience in disabilities and special needs or a related area, supervisory experience preferred.

Primarily first shift, Monday through Friday, 8 am – 5pm, some weekend and alternate shift work as needed.  Must carry a mobile phone to enable 24-hour contact.

Salary:  $29,543.70
