SERVICE COORDINATION
Service Coordination program will link the individual's needs with the most appropriate community resources. The Service Coordinator works together with the individual and their family to identify and meet their special needs. This may include referrals to the Adult Activity Program, Family Arranged Respite, and Residential Placement. The service Coordinator may also advocate for the individual with other agencies.

EARLY INTERVENTION
Early Intervention is a home base and family-centered program which serves children ages 0-6 with special needs, such as low birth-weight, developmental delay, visual or hearing impairment. The Early Interventionist works together with the family to help the child reach their full potential. The Early Interventionist also provides emotional support for families and coordinates appropriate services including transition to school and other developmental programs, which help the child, reach their potential.

INDIVIDUAL REHABILITATION SUPPORT
Individual Rehabilitation Support is intended to provide supports for independence, self-direction and participation in community activities. IRS includes assistance to develop & maintain functional abilities, personal identity, responsibilities, and self-determination. These services are non-facility based and are delivered in non-congregate settings.
